Beyond Singing: Enrollment

I understand that once my enrollment is confirmed, no refunds will be given for a student withdrawal from the whole course. However, if you give at least two-week notice to withdraw from the course, lesson credit at FCM will be offered instead. *

No Make-Ups will be offered for Student Absences from a class since this is a short course. If for any reason the instructor must reschedule, a Make-Up will be offered or Lesson Credit Given.*

I understand payment for the whole session is due upon Enrollment. We will send you an emailed invoice to complete payment for your enrollment. Your enrollment will be confirmed once payment is received. Current FCM Students receive a $10 Discount off the Total Course Amount.*

I acknowledge I must drop-off my child at the advertised start-time and pickup my child on time or early. If a parent is between 15-30 mins late for pickup, we will charge a $15 childcare fee. If a parent is between 30-60 mins late for pickup, we will charge a $25 childcare fee.*

I release, waive, discharge and covenant not to sue FCM, its staff, director, or employees, (hereinafter referred to as “Releasees”) from all liability to the undersigned, their personal representatives, heirs, minor child or children, and next of kin for any loss or damage, and any claim or demands therefore on account of injury to the person or property or resulting in illness and/or death of the undersigned, arising out of or related to the activities, whether known or unknown.*

FCM reserves the right to cancel the session if the Enrollment Minimum is not met. In the unlikely event of this happening, a full refund will be given or lesson credit offered.*
