Please fill out this form as best you can so we can update your profile in our database. All families with multiple students renewing will be charged a $35 flat renewal fee per family. If only one student is enrolled, the renewal fee is $25. (Renewal fees will be processed automatically to your payment method on file once you are scheduled for the Fall Semester).
I authorize FCM and any of its agents/employees to take my/my child's photo, video, or audio recording for marketing and/or publication (online, or otherwise).
I acknowledge that I must give a minimum of 24 hours notice when cancelling or rescheduling a lesson, otherwise the lesson will count as fulfilled.*
I acknowledge that my tuition is paid in equal installments each month for lessons in the Academic Year, unless I start later than Fall Week 1 (then your first payment will be pro-rated). The installment amount does not change despite absences or vacations. I understand a tuition installment is not for 4 lessons a month.*
